Tirzepatide (Subcutaneous): Overview
Tirzepatide is a once-weekly, subcutaneous prescription medication used to manage type 2 diabetes (with diet and exercise) and to support weight management for eligible adults. It is a dual incretin therapy that acts on GIP and GLP-1 receptors to improve glycemic control, reduce appetite, and promote weight loss.
Before Using Tirzepatide
Allergies & Hypersensitivity
Do not use if you have had a serious allergic reaction to tirzepatide or any component of the formulation.
Age Considerations
- Pediatric use: safety/efficacy not established.
- Older adults: generally appropriate; individualize based on comorbidities and sensitivity to GI effects.
Pregnancy & Breastfeeding
Use only if potential benefit justifies potential risk. Discuss planning for pregnancy and breastfeeding with your clinician.
Medical Conditions
- History of pancreatitis.
- Severe GI disease (e.g., gastroparesis).
- Kidney disease/dehydration risk.
- Personal/family history of medullary thyroid carcinoma (MTC) or MEN2.
- Diabetic retinopathy or vision changes.
Drug Interactions
- Other glucose-lowering drugs (insulin, sulfonylureas): ↑ hypoglycemia risk—may need dose adjustments.
- Oral contraceptives during dose escalation: consider back-up contraception (delayed gastric emptying can affect absorption).
- Drugs affected by gastric motility/absorption (e.g., warfarin and others): monitor as appropriate.
- Concurrent GLP-1 agents: typically not combined.
